Print and Fill Out the Forms!
Dear Parents,
Each registered boy and their parent must submit a medical form. We are going to keep these with Dr. Bobbi Stanley and we will get a satchel to carry them with us on our camp outings. The reason these are required is to make sure that if something happens we can provide information that may help in the event of an emergency. Because of medical privacy, Dr. Stanley will keep the set of forms for the Pack and will provide them to the Tour Leader who signs the Tour Permit for the outing.
In addition, there is a section C on the Form for Talent Release. This release form allows us to put pictures and video of your son on our website in the future.
Thanks for your help with this administrative task that is required by the Cub Scout organization. If anyone has questions about this, feel free to see me and talk with me about it.
